PROGRAM HELPS CANCER PATIENTS NAVIGATE CARE
|
CANANDAIGUA, NY- The Patient Navigation Toolkit in Cancer Care, which Thompson Health has been offering its breast cancer patients for almost a year, is now being distributed nationally. HANYS, as part of its Breast Cancer Demonstration Project, along with Thompson Health and several other hospitals, developed this valuable resource. The feedback received was so positive that HANYS shared the information with Pfizer who now provides these kits free of charge to health care facilities across the country.
The Patient Navigation program was designed to facilitate access to appropriate cancer-care services within a health care system.
"A cancer diagnosis often leads to a feeling of being totally overwhelmed. To help alleviate this, the toolkit allows patients easy access to the people and services available to help them," says Deborah Dannahe, Medical Social Worker at the Sands Cancer Center.
The toolkit includes helpful information such as where they can go for treatments, support groups, names of doctors and information on the test/procedures they will undergo. It allows the cancer patient to feel more in control of their care plan and addresses many of the unique issues they face.
